Facial Cosmetic Surgery

Facial Cosmetic Surgery:  Board Certified in Facial Plastic Surgery as well as Otolaryngology, Dino Madonna, MD uses the latest techniques to minimize scarring and down time in restoring your "look."  As a fellowship trained surgeon, Dr. Madonna offers surgical procedures, including Mohs Micrographic Surgery, in order to treat and reconstruct deformities which may have been affected by skin cancers or other traumas.

Mohs surgery offers the highest cure rate among treatments for skin cancer and can be used for practically any type of skin cancer. There are many other acceptable skin cancer treatment options including freezing (cryosurgery), scraping & burning, surgical removal (excision), and laser surgery, which may be appropriate alternatives but all require the surgeon to estimate how extensively to treat the area around the tumor. There are many advantages to Mohs surgery compared to other treatments including its high cure rate and cost-effectiveness. It is especially appropriate for skin cancers that:

  • Develop on areas where preserving cosmetic appearance and function are important
  • Have recurred after previous treatment or are likely to recur
  • Are located in scar tissue
  • Are large
  • Have edges that are ill-defined
  • Grow rapidly

 

Other Facial Cosmetic Surgery Procedures

  • Rhinoplasty (Surgery of the Nose):  Every year, half a million people who are interested in improving the appearance of their noses seek consultation with facial plastic surgeons. Some are unhappy with the noses they were born with, and some with the way aging has changed their nose. For others, an injury may have distorted the nose, or the goal may be improved breathing.  Because the nose is the most defining characteristic of the face, a slight alteration can greatly improve one's appearance.

  • Blepharoplasty (Surgery of the Eyelids):  Droopy eyelids can make you look older and can also impair vision.Every year, one hundred thousand men and women choose blepharoplasty to improve the way they look.

  • Facial Scar Revision:  When the skin is in the process of recovering from an injury, whether the result of an accident, surgery, a burn, or acne, scarring will occur wherever multiple layers of the skin have been affected. Once a scar forms, it is permanent but may be made less visible or relocated surgically.
  • Forehead Lift:  Your tired, angry, or sad expression may not reflect how you actually feel. As a result, many people have opted for a procedure known as the forehead lift.
  • Laser Surgery:  A laser is a high-energy beam of light that can selectively transfer its energy into tissue to treat the skin. Lasers contain a material that produces and amplifies light.  Many procedures cannot be done without the laser. Likewise, many procedures are better performed without the laser. Even with the sparkle, pure color, and high-energy beam, the laser is not always the best tool for surgery. The choice of using a laser or other surgical methods is carefully made by your Lake ENT facial plastic surgeon.
  • Mentoplasty (Surgery of the Chin):  A well defined chin helps give balance to the face and creates a major part of one's profile.  Surgeons who specialize in rhinoplasty, or surgery of the nose, are often the first to suggest that changes in chin size or shape may enhance a profile as much as rhinoplasty. It is common for the facial plastic surgeon to recommend chin surgery in addition to nose surgery when the surgeon sees that chin augmentation is necessary to achieve facial balance and harmony.
  • Otoplasty (Surgery of the Ears):  Children, long the victims of cruel nicknames, are the most likely candidates for otoplasty, but this surgery can be performed at any age after the ears have reached full size, usually around five to six years of age. Even if the ears are only mildly distorted, the condition can lead to self-consciousness and poor adaptation to school. When it comes to otoplasty, conventional wisdom is the earlier the better.
  • Rhytidectomy (Surgery of the Face):  Aging of the face is inevitable. As the years go by, the skin begins to loosen on the face and neck. Crow's feet appear at the corners of the eyes. Fine forehead lines become creases and then, gradually, deeper folds. The jawline softens into jowls, and beneath the chin, another chin or vertical folds appear at the front of the neck. Heredity, personal habits, the pull of gravity, and sun exposure contribute to the aging of the face. As the aging population grows, it is obvious why rhytidectomy has become the third most desired facial plastic surgical procedure.
  • Skin Resurfacing:  Premature wrinkling is more common now than ever before. Both men and women seek facial plastic surgery to reverse the effects of sun damaged skin. Surgery may also improve the pitted look of acne scars and remove pre-cancerous skin growths called keratoses.


 

 

Skin Cancer

According to the Skin Cancer Foundation, one in five Americans will be diagnosed with skin cancer – the most common form of cancer in the United States.

One of the most common and most frightening places for skin cancer to develop is on the face.  Today, most facial skin cancers can be removed accurately and safely using various procedures including the Mohs procedure. Using facial plastic and reconstructive surgical techniques, Lake ENT’s surgeons can repair or minimize disfigurement caused by the surgery.

About Mohs Micrographic Surgery

With Lake ENT & FPS physicians specially trained and certified, Mohs surgery is the most effective and advanced treatment for skin cancer today. It offers the highest potential for cure – even if the skin cancer has been previously treated by another method.

Local anesthesia is administered around the area of the tumor as the patient is awake during the entire procedure. The use of local anesthesia in Mohs surgery versus general anesthesia provides numerous benefits, including the prevention of lengthy recovery and possible side effects from general anesthesia. After the area has been numbed, the Mohs surgeon removes the visible tumor along with a thin layer of surrounding tissue. This tissue is prepared and put on slides by a technician and examined under a microscope by the Mohs surgeon. If there is evidence of cancer, another layer of tissue is taken from the area where the cancer was detected. This ensures that only cancerous tissue is removed during the procedure, minimizing the loss of healthy tissue. These steps are repeated until all samples are free of cancer.  While there are always exceptions to the rule, most tumors require 1 to 3 stages for complete removal.

When the surgery is complete, the Mohs surgeon will assess the wound and discuss options for ideal functional and cosmetic reconstruction if reconstruction is necessary.

Ear

Conditions that affect the health and function of the ear can be as simple as ear wax or something more serious.  Lake ENT offers a full range of diagnostic and treatment services for the entire family to help determine the cause of the condition as well as most suitable treatment.   

When hearing loss is part of your diagnosis, Lake ENT’s physicians work closely with our Audiology team to provide seamless care and vice-versa.  We also utilize the resources of our balance and vestibular specialists to address any inner ear conditions that may be causing dizziness or vertigo.

 

Audiology

Conditions that affect hearing loss can occur at any age – from birth to Boomer and beyond.  Lake ENT offers extensive diagnostic and testing resources including a hearing test booth at each of our locations.  Our nationally certified Doctors of Audiology, licensed hearing aid specialists, and audiology staff work together, along with the medical staff, to zero-in on the cause, degree and most effective treatment for your hearing loss.

Hearing aids may be in-the-ear (ITE), behind-the-ear, (BTE), in-the-canal (ITC) or completely-in-the-canal (CIC).

Hearing aids are custom fit for each wearer. Behind-the-ear aids fit behind the ear by means of a custom ear mold. Our office carries both standard hearing aids as well as the newer computer-programmable models and even those that now come with Bluetooth technology.  In addition, we also service all hearing aids, and carry a variety of hearing aid batteries.

Once you are fitted for a hearing aid at Lake ENT, you have a partner for life in insuring the fit, effectiveness and maintenance of your device is always at its maximum.

 

Balance / Vestibular Conditions

According to the Centers for Disease Control, each year, one in every three adults age 65 and older falls. Falls can lead to moderate to severe injuries, such as hip fractures and head traumas, and can even increase the risk of early death. Fortunately, falls are a public health problem that is largely preventable.

Research – as reported by Consumer Reports – indicates that 1 in 3 adults over age 40 had signs of inner-ear dysfunction. This translates to 69 million people nationwide. And nearly a third of the people in this group (more than 22 million people) were unaware of their balance problems, as they'd had no symptoms suggesting anything was wrong. These people were three times more likely to have a serious fall than those with a healthy sense of balance. And the risk of falls was 12 times higher among those with inner-ear problems who'd experienced dizziness or other symptoms.

Preventing falls, particularly those due to inner ear and balance conditions, begins with early and proper diagnosis followed by treatment which may include medications, lifestyle modifications and/or physical therapy.

Nose / Sinuses

As our name would suggest, Lake Ear, NOSE, throat & Facial Plastic Surgery Associates knows noses.

Congestion due to cold, flu or allergy, allergic rhinitis, sinus infection, a deviated septum, sinus pressure and related headaches lead the list of conditions often seen at our practice.  

Each year these conditions lead to loss productivity at both work and school.  A 2009 edition of the American Journal of Rhinology & Allergy states that the average chronic sinusitis sufferer misses 28-51% more work days than the non-chronic sinusitis sufferer.

Left untreated, either through medication or surgery, many of these conditions can lead to further discomfort, adverse affects on lifestyle and more serious medical conditions.

 

Sleep Disorders

The American Sleep Apnea Association explains that many sleep disorders can be attributed to sleep apnea.  

Obstructive sleep apnea (OSA) is caused by a blockage of the airway, usually when the soft tissue in the rear of the throat collapses and closes during sleep. In central sleep apnea, the airway is not blocked but the brain fails to signal the muscles to breathe. Mixed apnea, as the name implies, is a combination of the two. With each apnea event, the brain briefly arouses people with sleep apnea in order for them to resume breathing, but consequently sleep is extremely fragmented and of poor quality.  

Because of the close association between the nose, throat, apnea and the resulting sleep disorders, Lake ENT provides diagnostic, testing and surgical, as well as non-surgical, solutions to help you get a better and safer night’s sleep.

 

Allergies

Whether seasonal or year-round, Central Florida provides a wide variety of environmental allergens.

Through in-office testing, Lake ENT’s allergy specialists identify environmental irritants causing your symptoms and then develop a plan of treatment.   Such treatment may include medications or a program of desensitization – a series of weekly injections.  Serums can be custom-made to your specific needs in our office.


 

Throat

Conditions of the throat can be a as simple as a minor nuisance or a major ordeal. Tonsillitis, strep throat, voice disorders, and even hoarseness all interfere with our ability to communicate. Many of these conditions can be improved or corrected with the care of one of our practitioners at Lake ENT.

While always a last option, we are extremely versed in the surgical treatments sometimes required to resolve such issues.  The removal of tonsils and adenoids may be part of your treatment.  Minimally invasive procedures employed by the physicians at Lake ENT has reduced discomfort and recovery time from months to, in some cases, days.

 

Head / Neck Cancers

The American Academy of Otolaryngology – Head and Neck Surgery reports that this year, more than 55,000 Americans will develop cancer of the head and neck (most of which is preventable); nearly 13,000 of them will die from it.

Many of the symptoms associated with head and neck cancers can also be associated with non-cancerous conditions.  But you can’t tell without an examination.  The physicians at Lake ENT can diagnose the condition to determine whether the symptoms are cancerous or not.  
Remember—when found early, most cancers in the head and neck can be cured with few side effects. Cure rates for these cancers could be greatly improved if people would seek medical advice as soon as possible.  If you detect warning signs of head and neck cancer, see your doctor immediately.

 

Thyroid / Parathyroid

Your thyroid gland is one of the endocrine glands that makes hormones to regulate physiological functions in your body, like metabolism (heart rate, sweating, energy consumed). Other endocrine glands include the pituitary, adrenal, and parathyroid glands and specialized cells within the pancreas.

The most common thyroid disorders include; An overactive gland, called hyperthyroidism , 
an underactive gland, called hypothyroidism and thyroid enlargement due to overactivity (as in Graves’ disease) or from under-activity (as in hypothyroidism).  An enlarged thyroid gland is often called a “goiter.”

The physicians at Lake ENT can determine whether you do have a thyroid condition and what the most appropriate treatment may be.  Such treatments offered include medications or surgery.
